A CPP Disability medical report. An insurer's own PDF. The fourth lab requisition of the morning. Each one asks for the same patient information in a slightly different arrangement, and each one interrupts clinical work to become data entry. FormsAI turns that around: upload the form, and the AI maps its fields, pulls the answers from the chart, and drafts the clinical sections for review.
It handles both kinds of PDF — proper fillable AcroForms and flat scanned layouts, where fields are detected and placed as an overlay you can adjust visually. Signature stamps land where they should. Batch mode fills the same form for a list of patients in one pass.
Privacy is a design constraint, not a checkbox: by default the drafting runs on self-hosted models on the clinic's own GPUs, and if a practice chooses to enable cloud models, requests pass through a PHI de-identification pipeline first. Sections the AI can't answer confidently are flagged for the physician rather than filled with guesses — the physician always signs a document they've reviewed.
